Appointment reschedules or cancellations

If you cancel your appointment less than 48 hours in advance, you will need to text your provider (Marilizabeth/623.693.1584 or Stephanie/623.693.1647) to ensure they are aware of the cancellation. If provider does not receive text (directly at number provided) you will be considered a no show or late cancellation. Per policy as of 11/15/24: if you cancel less than 48 hours in advance you will automatically be charged $110.00. All patients must have a credit card on file and by signing below, you understand and agree to being charged the $110.00 no show/late cancellation fee.

No-show or late arrival

If you are more than 15 minutes late for an appointment, you will be charged $110.

Medication refills

Please allow 4-5 business for medication refill requests to be completed.
