Insurance Coverage
It is up to the individual patient to verify with his/her individual insurance provider to verify coverage for treatment with the individual providers and services rendered including referrals.
Appointment reschedules or cancellations
If you cancel your appointment less than 48 hours in advance, you will need to text your provider (Marilizabeth/623.693.1584 or Stephanie/480.300.4761 to ensure they are aware of the cancellation. If the provider does not receive text (directly at number provided) you will be considered a no show or late cancellation. Per policy as of 11/15/24: if you cancel less than 48 hours in advance you will automatically be charged $110.00. All patients must have a credit card on file and by signing below, you understand and agree to being charged the $110.00 no show/late cancellation fee.
No-show or late arrival
If you are more than 15 minutes late for an appointment, you will be charged $110.
Medication refills
Please allow 4-5 business for medication refill requests to be completed.
